Patient confidentiality, rights, responsibilities and privacy
In accordance with PHIPA, Yorkview Medical Centre collects personal health information directly from you (or the person acting on your behalf). The type of information we collect includes:
* Your name, date of birth, OHIP # and address
* Your health history
* Records of your visits to Yorkview Medical Centre (including the care that you received during those visits)
At Yorkview Medical Centre, it is our responsibility to keep your personal health information private and confidential. We ensure that our practices are in compliance with Ontario’s Personal Health Information Protection Act, 2004 (PHIPA) in order to protect our patients’ privacy while providing effective healthcare services.
Cancellation Policy
We understand that there are times when you have to miss an appointment due to emergencies or obligations for work or family. If you need to reschedule or cancel an appointment, we require a minimum of 24 hours notice so that we can provide that appointment slot to another patient.
Any missed pre-booked periodic health exam appointment without a 24 hour (1 business day) cancellation notice will be invoiced $100 (adults) and $50 (children). All other missed appointments will be invoiced $50.
Uninsured Services
Yorkview Medical Centre provides a number of uninsured services. Patients may opt to pay individually for these services or to participate in the Yorkview Medical Centre FHO Plan for Uninsured Services.
