Annual Uninsured Services Plan Fees

Family of 2 or more: $250

Individual: $125

Senior-Individual: $50

Senior-Couple: $100

Services Included In Plan

Sick/Back to Work Notes: $25

Insurance required referral/prescription for massage, physiotherapy, chiropractic etc: $25

Copy of results/consults/reports etc. (printed or emailed): $10 per request

School/Camp/Day Care forms: $25

Fitness Certifications (for work or other): $25

EI Medical Certificate/Parental Leave Certificate: $25

Employer required forms, eg sickness/short term disability/functional ability forms: $50-$100

Travel Cancellation form: $100

Immunization Summary: $25

Medication special authorization form: $50

Letter on behalf of patient: $50 to $200

Procedures

Removal of skin tags/sebaceous keratosis: $50 for first and $5 for each additional

Removal of sebaceous cysts: $175 for one and $75 each additional
